About the Role:

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team as a Community Care Specialist. In this role, you will provide high-quality nursing care to patients in their homes, working with patients of all ages and complexity of care needs.

Key Responsibilities:

  • Participate in scheduled visits to assess patient needs and provide care within your scope of practice
  • Provide safe and ethical nursing care, using appropriate assessment tools and techniques to assess patients' individual needs
  • Develop and implement care plans, monitoring and adjusting as required
  • Engage patients using a collaborative approach, building rapport and trust
  • Maintain and update professional patient records related to patient encounters and plans

Requirements:

  • Registration with the NSCN as a Licensed Practical Nurse in good standing
  • Valid driver's license and access to a vehicle
  • Experience in delivering community care is an asset
  • Reliability and flexibility
  • Willingness to travel to and from patient residences within a designated area
  • Experience working with vulnerable populations
  • Passion for making a difference in your community
