About the Job:

We are seeking a compassionate and dedicated Licensed Practical Nurse to join our Transitional Care Centre team in West Bedford, Nova Scotia. As a Licensed Practical Nurse, you will play a vital role in providing high-quality care to patients who require additional time, services, and support to transition back to their previous living situation.

About the Role:

  • Perform initial and regular health assessments to maintain patient quality of life.
  • Support development of individualized care plans that focus on the patient's functional abilities and support network.
  • Assess, plan, implement, and evaluate nursing care of patients.
  • Ensure compliance with Philosophy of Service and patient care plan to ensure optimal health and wellness for each patient.
  • Manage patient medication in accordance with legislative requirements.
  • Administer and record prescribed medications in accordance with facility policy.
  • Provide consultation regarding patient issues and standards of care to interdisciplinary team.
  • Communicate with all other departmental teams to increase awareness of patient health changes to ensure effective service.
  • Ensure all completed care tasks are documented appropriately using identified technology and tools.
  • Support a commitment to ongoing quality improvement, continuous exploration and implementation of new approaches, learning, teaching, and mentoring.

About You:

  • A Diploma in Practical Nursing and current registration with the Nova Scotia College of Nursing.
  • Comprehensive knowledge of nursing and health care practices, excellent communication, and interpersonal skills.
  • Availability to work a schedule of flexible hours and all shifts inclusive of days, evenings, nights, weekends, and holidays.
  • Advanced proficiency in the English language is required, proficiency in other languages considered an asset.
  • Ability to provide a CPR & Emergency First Aid Certificate, Clear Criminal Record Check and Vulnerable Sector Screening upon hire.
  • A passion for the healthcare sector and/or ensuring seniors have access to quality accommodations, services, and care.
