We are seeking a skilled and compassionate Licensed Practical Nurse to join our Transitional Care Centre team in West Bedford, Nova Scotia.
As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and supporting the transition of patients from hospital to home or community.
Key Responsibilities- Perform initial and regular health assessments to maintain patient quality of life
- Support the development of individualized care plans that focus on the patient's functional abilities and support network
- Assess, plan, implement, and evaluate nursing care of patients
- Ensure compliance with our Philosophy of Service and patient care plan to ensure optimal health and wellness for each patient
- Manage patient medication in accordance with legislative requirements
- Administer and record prescribed medications in accordance with facility policy
- Provide consultation regarding patient issues and standards of care to the interdisciplinary team
- Communicate with all other departmental teams to increase awareness of patient health changes
- Ensure all completed care tasks are documented appropriately using identified technology and tools
We are looking for a candidate who:
- Has a Diploma in Practical Nursing and is currently registered with the Nova Scotia College of Nursing
- Has comprehensive knowledge of nursing and health care practices, excellent communication, and interpersonal skills
- Is available to work a schedule of flexible hours and all shifts, including days, evenings, nights, weekends, and holidays
- Has advanced proficiency in the English language
- Can provide a CPR & Emergency First Aid Certificate, Clear Criminal Record Check, and Vulnerable Sector Screening upon hire
