About the Role

CarePartners is a leading provider of home health care services in Ontario, seeking a skilled and compassionate Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our team as a Visiting Nurse.

Key Responsibilities
  • Develop and maintain effective care plans for patients in the community, working independently or in collaboration with the Home and Community Care Support Services (HCCSS).
  • Provide exceptional nursing services to patients in their homes, including one-on-one care and virtual consultations.
  • Communicate and liaise with patients, family members, and other members of the interdisciplinary care team to ensure seamless care coordination.
  • Perform other clinical duties as required, including administering medications and providing wound care.
Requirements
  • Current registration with the College of Nurses of Ontario (CNO) in good standing.
  • Strong desire and commitment to making a difference in the lives of patients.
  • Exceptional interpersonal skills and ability to work effectively in a team environment.
  • Valid driver's license and vehicle, as regular travel is required.
  • Excellent English verbal and written communication skills.
  • CPR certification and current immunization record, including a 2-step TB test.
  • Current vulnerable sector check and clear background check.
