We are seeking a skilled Registered Practical Nurse to join our team at RT Medical. As a key member of our healthcare team, you will provide high-quality patient care and support to individuals in their homes, clinics, and other healthcare settings.
Key Responsibilities- Assessment and Planning: Conduct thorough assessments of patients' needs and develop personalized care plans in collaboration with patients, families, and healthcare professionals.
- Care Delivery: Provide direct patient care, including wound management, IV medications, and other clinical duties as required.
- Communication and Coordination: Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination and patient advocacy.
- Travel and Flexibility: Travel within your region to provide care to patients in their homes, clinics, and other settings, and be flexible to meet the changing needs of our patients.
- Education and Certification: Diploma from a recognized RPN program or equivalent, and registration with the College of Nurses of Ontario.
- Experience and Skills: Experience as a visiting nurse, community nurse, or in a similar role, with excellent communication, problem-solving, and time management skills.
- Physical Demands: Ability to stand, bend, walk, crouch, kneel, and reach, with the ability to lift 50lbs (23 kg) independently.
- Additional Requirements: Valid Standard First Aid and Level C CPR certification, updated immunization records, and a clear criminal background check.
