Job Description for Nurse Care Specialist

At RT Medical, we're committed to providing high-quality patient care services. As a Nurse Care Specialist, you will play a vital role in delivering compassionate and personalized care to our patients in their homes, clinics, and other settings.

Key Responsibilities:
  • Assess and Plan Care: Conduct thorough assessments of patients' needs and develop individualized care plans to meet their unique requirements.
  • Provide Skilled Nursing Care: Deliver high-quality nursing care to patients, including wound care, IV medications, and re-sites, catheter care, compression/VAC, and ostomy care.
  • Collaborate with Healthcare Teams: Work closely with patients, families, caregivers, and other healthcare professionals to ensure seamless care coordination and effective communication.
  • Travel and Flexibility: Travel within your region to provide care to patients in their homes, clinics, and other settings, and be flexible to adapt to changing patient needs.
  • Documentation and Reporting: Maintain accurate and detailed records of patient care, including assessments, interventions, and outcomes.
Requirements:
  • Qualifications: Bachelor of Science in Nursing degree (BScN), Bachelor of Nursing degree (BN), or Registered Nurse or Registered Practical Nurse in good standing with College of Nurses of Ontario.
  • Experience: Experience as a visiting nurse, visiting registered nurse, visiting registered practical nurse, and/or community nurse is considered an asset.
  • Physical Abilities: Ability to stand, bend, walk, crouch, kneel, and reach, and lift 50lbs (23 kg) independently.
  • Certifications: Valid Standard First Aid and Level C CPR, updated immunization records, and a clear criminal background check.
