  • Complete follow-up documentation for individuals after emergency room or hospital visits.
  • Review, annotate, initial, and date lab results, x-rays, and consultations; follow up with practitioners as needed.
  • Provide timely documentation regarding home visits as per established guidelines.
  • Adhere to pressure wound policies and documentation standards.

Deliver essential nursing services and oversight to Residential/Day Hab staff.



  • Respond promptly to emergency requests, both within and outside your caseload.
  • Conduct nursing assessments and maintain related documentation, including RN notes and nursing logs, thoroughly and within set timeframes.
  • Implement individual-specific nursing interventions.
  • Address staff inquiries regarding individual-specific medical and nursing matters.
  • Ensure that Plans of Nursing Services (PONS) are current and signed by staff, maintaining them in digital files.
  • Provide health-related training and consultation to day program and residential staff.
  • Annually review the medical section of Individualized Service Plans (ISP) and conduct medication audits for each home.
  • Stay informed about updates to the medical section of the ISP.
  • Obtain informed consent from individuals for medications, following established policies.

Ensure adequate nursing assessment and supervision of Direct Support Assistants (DSAs) regarding individual-specific nursing delegated tasks, including medication administration.



  • Complete initial medication certification and perform annual clinical performance evaluations for DSAs on procedures, including medication administration.
  • Provide training for staff on new or revised medication policies as needed.
  • Timely submit certification and recertification information for database maintenance.
  • Document medication errors and oversee staff remediation as required.

Provide clinical supervision to Licensed Practical Nurses (LPNs).



  • Ensure adequate nursing supervision and complete performance evaluations regarding individual-specific nursing procedures.

Maintain professional competencies:


  • Participate in Nursing Discipline meetings.
  • Complete mandated training and available discipline-specific training online.
  • Identify opportunities for personal and departmental improvement and contribute to action plans.
  • Engage in special projects and assignments as needed.
