About the Role:

Your Responsibilities:
  • Conduct and coordinate assessments to inform treatment strategies.
  • Deliver case management services, including planning, coordination, and evaluation of client progress.
  • Facilitate individual and group therapy sessions utilizing evidence-based methodologies such as Motivational Interviewing, Trauma-Informed Care, and Cognitive Behavioral Therapy.
  • Provide support and counseling to family members, and lead educational group sessions.
  • Work closely with physicians and the clinical team to ensure a cohesive patient experience through regular meetings to review treatment plans and progress.
  • Oversee discharge planning and connect clients with appropriate community resources.
  • Identify discharge requirements based on comprehensive assessments and care plans.
  • Lead the establishment of treatment objectives and collaborate with clients and team members to create actionable steps.
  • Monitor and evaluate treatment plan progress as a team.
  • Serve as the communication liaison among clients, the clinical team, and external partners, ensuring critical information is shared effectively.
  • Maintain accurate documentation and record-keeping in accordance with organizational policies.
Qualifications:
  • Bachelor's or master's degree in a recognized Social Science discipline with a focus on addiction.
  • Membership in good standing with the relevant regulatory body in Ontario or in pursuit of necessary credentials.
  • Proficient in delivering evidence-based treatments for complex mental health issues and substance use disorders.
  • Demonstrated experience in assessment and treatment using evidence-informed practices.
  • Strong communication and interpersonal skills.
  • Understanding of measurement-based care to support clinical decision-making.
  • Ability to lead and coordinate an interdisciplinary team effectively.
  • Experience in individual and group counseling, including the ability to present educational content to larger audiences.
  • Capacity to implement inclusive and culturally sensitive approaches.
  • Knowledge of addiction issues across a continuum of care.
  • Valid Ontario driver's license and access to a vehicle as needed.
  • Familiarity with relevant legislation such as the Mental Health Act and Health Care Consent Act.
