Registered Nurses play a vital role in enhancing the lives of their clients by addressing a wide array of care requirements within the framework of SJHC's policies and the standards set by the College of Nurses of Ontario. Through their dedicated efforts, Registered Nurses foster the healing and autonomy of their clients while promoting the ongoing enhancement of community health.CORE DUTIES AND RESPONSIBILITIESEffectively employs the nursing process to deliver superior Wound and Ostomy Care for clients and/or their caregivers.Collaborates with the interdisciplinary health team to ensure cost-effective, high-quality Wound Care, adhering to Best Practices.Integrates current knowledge and research regarding Wound Care Protocol into practice.In partnership with the physician, CCAC Case Manager, ICCP coordinators, and Primary Nurse, formulates a comprehensive care plan for patients requiring wound care in the community.Initiates and continuously adjusts the nursing care plan in consultation with the client and/or caregiver.Encourages the independence of clients and caregivers by considering their unique needs, strengths, and challenges.Utilizes available resources to facilitate problem-solving, promote participation in decision-making, and ensure access to appropriate services.Engages in the evaluation and enhancement of the Wound Care Protocol and overall program.Contributes to the education of other SJHC team members regarding the protocol and product usage, providing guidance on optimal treatment options.Maintains regular communication with supervisors to discuss issues or unique challenges and exchange feedback.Participates in bi-weekly nursing case rounds.CORE COMPETENCIESConsistently embodies our mission, vision, and values: Supporting compassionate care, faith, and diversity; delivering quality care; pursuing and sharing knowledge; respecting diversity; and remaining faithful.Ability to collaborate effectively with others, sharing expertise and leveraging the expertise of colleagues.Actively fosters a healthy, supportive, and inclusive work environment.Proactively contributes to initiatives that support and encourage positive change.Exhibits strong problem-solving skills, with logical and analytical thinking capabilities.Demonstrates creativity in developing solutions to complex challenges.Adopts a solid team-based approach to daily work activities.Possesses superior verbal and written communication skills, with a strong customer service orientation.Maintains a positive and professional demeanor, with a proactive attitude.Willingness to go above and beyond to support colleagues.Exhibits leadership and mentoring abilities to motivate and assist others in their development.Strong understanding of quality outcomes and process efficiencies.Exceptional organizational and planning skills.Proven attention to detail.Ensures ongoing compliance with corporate policies, health and safety regulations, and relevant external standards.Actively participates in both internal and external training requirements.Engages fully in corporate quality initiatives.Upholds the privacy and confidentiality of all personal health records, adhering to all privacy requirements related to electronic medical record systems.KEY EDUCATION AND EXPERIENCEBachelor of Science Degree from an accredited institution, required.Current and unrestricted Registered Nurse license, required.Member in good standing with the Ontario College of Nurses, required.Valid Ontario driver's license in good standing, required.Proficient in MS Office software, including Excel spreadsheets, required.Knowledge of community support systems, Home Care, Acute, and Long-Term Care, required.Familiarity with Procura software is preferred.Certification as a wound care nurse or equivalent courses; IIWCC course recommended.Minimum of one (1) year of recent experience in home care is preferred.Demonstrated excellence in direct client care.Outstanding verbal, written communication, and interpersonal skills.
