Position Title: Community Health Program Coordinator

Key Responsibilities:

  • Collaborate with the Occupational Therapist to oversee the daily operations of the Health Equipment Loan Program, which includes the Specialized Equipment Program and the Transitional Care Unit pilot expansion. Assist with the cleaning of equipment and perform service center tasks as directed by the onsite Service Center Coordinator.
  • Establish and nurture relationships with hospitals and long-term care facilities to ensure effective delivery of the Health Equipment Loan Program, including the logistics of equipment delivery and retrieval.
  • Maintain accurate inventory records related to equipment loans, returns, and program statistics in compliance with established policies.
  • Manage administrative tasks, ensuring that financial transactions are processed accurately and that public information is communicated promptly.

Qualifications:

  • A minimum of a post-secondary education or diploma in a health-related discipline, coupled with one to two years of relevant work experience, or an equivalent combination of education and experience in health, customer service, and administration.
  • Experience as an Occupational Therapy Assistant or Rehabilitation Assistant is advantageous.
  • Ability to effectively prioritize and manage multiple tasks in a dynamic client-focused environment.
  • Strong decision-making skills and exceptional organizational abilities are essential.
  • Proficient communication skills, both written and verbal, in English are required.
  • Demonstrated proficiency in computer applications and administrative functions.
  • Previous experience working with volunteers and seniors is considered beneficial.
  • Capability to perform medium to heavy lifting as needed.

Working Conditions:

  • A valid driver's license is necessary for travel within the region, along with a satisfactory driver's abstract.
  • Candidates must be eligible to work in Canada. If you are not a citizen or permanent resident, please review your visa eligibility for this position.
  • Successful candidates will undergo a pre-employment screening process, which includes a satisfactory Enhanced Police Information Check (E-PIC).
