Job Summary

The successful candidate will work closely with our residents, healthcare team, and family members to develop and implement individualized treatment plans that promote independence, mobility, and overall well-being.

Key Responsibilities
  • Conduct OT/ADP Assessments: Conduct thorough assessments of residents' physical and cognitive abilities to identify areas for improvement and develop effective treatment plans.
  • Develop Individualized Treatment Plans: Collaborate with residents, family members, and healthcare team to create personalized treatment plans that address their unique needs and goals.
  • Document Progress Notes: Accurately document all progress notes in our facility's health record system to ensure seamless communication and collaboration among team members.
  • Complete Feeding Assessments: Conduct thorough assessments of residents' feeding abilities to identify areas for improvement and develop effective strategies for enhancing their nutrition and hydration.
  • Assess Falls Prevention and Least Restraints: Conduct thorough assessments of residents' risk for falls and develop effective strategies for preventing falls and reducing the use of restraints.
  • Collaborate with Multidisciplinary Team: Work closely with our physiotherapist, nursing staff, and other healthcare professionals to develop and implement comprehensive treatment plans that address the unique needs of our residents.
Qualifications
  • Registration and Licensure: Must be registered and in good standing with the College of Occupational Therapists of Ontario and possess a valid ADP Authorizer.
  • Professional License Insurance: Must have current professional liability insurance.
  • Excellent Interpersonal and Communication Skills: Possess excellent interpersonal and communication skills to effectively interact with residents, family members, and healthcare team.
  • Strong Written and Verbal Communication Skills: Possess strong written and verbal communication skills to effectively document progress notes and communicate with team members.
  • Experience in Long Term Care: Experience working in a long term care home is a definite asset.
  • Current Police Check: Must have a current police check for the vulnerable sector.
