We are seeking a highly skilled and compassionate Nurse Clinician Case Manager to join our Neuromotor Program team at BC Children's Hospital. As a key member of our interdisciplinary team, you will provide expert care and support to children and families affected by neuromotor conditions.
Key Responsibilities- Initiate and coordinate assessments, respond to patient and family inquiries, and provide anticipatory guidance and case management.
- Direct the intake/access process for the Neuromotor Program team, including receiving and documenting child-specific information, conducting interviews, and assessing referral urgency and appropriateness.
- Collaborate with healthcare team members to evaluate family-centered patient management, review patient and family history, and develop individualized service plans.
- Coordinate teaching and education for patients and families with new neuromotor condition diagnoses, including developing teaching resources and referring to other healthcare professionals.
- Provide supportive case management to families, ensuring they receive assistance with patient advocacy, program information, and community health and educational resources.
- Act as a liaison between the program, interdisciplinary team, and community health and educational services to clarify information and ensure patient and family needs are addressed.
- Establish a collaborative working relationship with Sunny Hill and BCCH staff and community-based healthcare providers.
- Liaise with community and professional groups regarding neuromotor conditions through consultation and teaching.
- Develop policies, procedures, and operational requirements related to the Neuromotor Program team, observing service needs and providing plans to address patient and family needs, clinic operations, and resource utilization.
- Participate in quality management activities related to the Neuromotor Program and nursing practice, assessing, planning, implementing, and evaluating strategic interventions to improve practice.
- Collaborate with the interdisciplinary team and nursing colleagues on research-oriented opportunities.
- Act as a clinical nursing resource for patients, families, nursing staff, healthcare professionals, and community agencies.
- Use the nursing process and specialized knowledge of neuromotor conditions and interventions to provide direct patient care and case management, with ongoing follow-up through telephone contact and individual appointments.
- Graduation from an approved School of Nursing with current practicing registration as an RN with the British Columbia College of Nursing Professionals (BCCNP).
- Three years recent related and progressively responsible experience, including previous experience working with the population and/or in a child development and rehabilitation setting, or an equivalent combination of education, training, and experience.
- CPR Training - Basic Cardiac Life Support (BCLS) - Level I.
- Valid BC Driver's License and access to vehicle and ability to travel.
