Job Summary

We are seeking a skilled Social Worker II to join our interdisciplinary team. As a Social Worker II, you will provide a range of social work services to patients and their families, including psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments to identify client needs and develop individualized plans
  • Provide individual, group, and family therapy to promote client well-being and resilience
  • Collaborate with interprofessional team members and community agencies to ensure comprehensive care
  • Develop and implement resource-based interventions to support client needs
  • Conduct risk screening and develop strategies to mitigate risks
  • Provide case management and coordination to ensure seamless transitions and discharge planning
Requirements
  • Completion of a bachelor's degree in Social Work
  • Active or eligible for registration with the Alberta College of Social Workers (ACSW)
  • Experience working in Addiction & Mental Health
  • Experience with Connect Care
