Job Title: Outreach Nurse, Provincial Assessment Centre

We are seeking a highly skilled and compassionate Outreach Nurse to join our team at the Provincial Assessment Centre. As an Outreach Nurse, you will play a critical role in providing high-quality care to individuals with complex mental health and substance use challenges.

Key Responsibilities:
  • Conduct comprehensive biopsychosocial assessments to identify client needs and develop individualized treatment plans.
  • Provide community-based case coordination and advocacy services to ensure continuity of care and support.
  • Maintain accurate and concise documentation of client information and treatment plans.
  • Collaborate with multidisciplinary teams to provide holistic care and support to clients.
  • Develop and maintain relationships with community resources and services to ensure access to necessary support and care.
Requirements:
  • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses & Midwives (BCCNM).
  • Two (2) years recent, related experience working with individuals in the community with severe and persistent psychiatric illness.
  • Valid BC Driver's License.
