We are seeking a highly skilled and compassionate Mental Health Support Worker to join our team at the Provincial Assessment Centre. As a key member of our interdisciplinary team, you will play a vital role in providing support and care to individuals with complex mental health and substance use challenges.
Key Responsibilities- Monitor clients' progress and provide emotional support and counseling to help them achieve their treatment goals.
- Participate in the admission, case planning, and discharge process for clients, including conducting interviews and gathering relevant information.
- Establish rapport with clients and provide guidance on daily living activities, such as time management and interpersonal communication.
- Coordinate services with medical professionals and community resources to ensure clients receive comprehensive care.
- Plan and lead groups, such as sports and exercise, social support groups, and social activities, to support clients' skill development.
- Prepare and maintain client charts, including updating files and completing standard forms.
- Diploma from a recognized Social Services Support Worker educational program or equivalent combination of education and experience.
- Valid driver's license and ability to travel locally.
- Knowledge of recovery principles, community resources, and behavior management.
- Ability to establish and maintain rapport with clients, observe changes in behavior, and deal effectively with conflict situations.
- Commitment to upholding the shared responsibility of creating lasting and meaningful reconciliation in Canada.
