Senior Persons Living Connected (SPLC) is dedicated to empowering seniors and fostering inclusive communities where they can thrive. We are seeking a compassionate and skilled Occupational Therapist to join our Geriatric Assessment Intervention Network (GAIN) Community Team.

As a key member of the interdisciplinary GAIN team, you will play a vital role in providing comprehensive assessments and developing individualized care plans for older adults facing various physical, emotional, and psychological challenges associated with aging.

Your Responsibilities:
  • Participate as a geriatric assessor in inter-professional Comprehensive Geriatric Assessments (CGAs), conducting clinical triage for new referrals and gathering data across all domains of the CGA.
  • Provide direct clinical care and consultation services to frail older adults, analyzing their capabilities and expectations related to daily living activities through observation, interviews, and formal assessments. Collaborate with the GAIN team to establish personalized care plans.
Your Qualifications:
  • University Degree in Occupational Therapy
  • Registered and in good standing with the College of Occupational Therapists of Ontario
  • Registration with CAOT (Canadian Association of Occupational Therapists) or OSOT (Ontario Society of Occupational Therapists) is an asset
  • Current CPR and First Aid Certification
  • Two to three years of experience working in the field, demonstrating extensive knowledge and skill in supporting individuals with dementia, delirium, mental health conditions, addictions, responsive behaviors, neurological disorders, and acute behavioral changes.
  • Experience and knowledge of working therapeutically with geriatric populations is highly valued.
  • Sound knowledge of cognitive and mobility assessment tools. Certification in Gentle Persuasive Approaches, PIECES or Montessori is preferred.
  • Skilled in crisis intervention and assessment.
  • Initiative, adaptability, excellent communication skills (written and verbal) in English.
  • Proficiency in other languages (Armenian, Greek, Mandarin, Tamil, Hindi, Urdu etc.) is desired.
