Job Summary

We are seeking a highly skilled Occupational Therapist to join our Geriatric Assessment Intervention Network (GAIN) Community Team at Senior Persons Living Connected. The successful candidate will be responsible for providing assessment and intervention services to older adults and seniors affected by the emotional, psychological, and physical impacts of aging.

Key Responsibilities
  • Participate in inter-professional Comprehensive Geriatric Assessment (CGA) as a geriatric assessor
  • Complete clinical triage for new referrals and gather data within all domains of CGA
  • Provide clinical care and consultation services to improve and/or maintain activities of daily living for frail older adults and seniors
  • Analyze capabilities and expectations related to life activities through observation, interviews, and formal assessments
  • Establish care plans for patients in collaboration with the GAIN inter-professional team
Requirements
  • University Degree in Occupational Therapy
  • Registered and in good standing with the College of Occupational Therapists of Ontario
  • Registration with CAOT (Canadian Association of Occupational Therapists) or OSOT (Ontario Society of Occupational Therapists) an asset
  • Current CPR and First Aid Certification
  • Two to three (2-3) years' working experience in the field
  • Demonstrated extensive knowledge and skill working with individuals with various types of dementia, delirium, mental health, addictions, responsive behaviours, other neurological conditions, and those who experience an acute behavioural change
  • Experience and knowledge of working therapeutically with geriatric populations an asset
  • Demonstrated sound knowledge of cognitive and mobility assessment tools
  • Certification in Gentle Persuasive Approaches, PIECES or Montessori preferred
  • Skilled at crisis intervention and assessment preferred
  • Initiative and adaptability skills
  • Excellent communication skills (written and verbal) in English
  • Proficiency in other languages desired (Armenian, Greek, Mandarin, Tamil, Hindi, Urdu etc.)
