Key Responsibilities
  1. Conduct thorough assessments of patients' functional abilities and develop personalized treatment plans to address their needs.
  2. Implement evidence-based interventions to improve patients' mobility, balance, and overall functional capacity.
  3. Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  4. Provide education and training to patients, families, and caregivers on safe and effective use of assistive devices and techniques.
  5. Document patient progress and outcomes in a clear and concise manner, adhering to regulatory and accreditation standards.
  6. Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements
  • Bachelor's or Master's degree in Occupational Therapy.
  • Current licensure/certification in the state of practice.
  • Experience working with geriatric patients, preferably in a PACE program setting.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
