Key Responsibilities- Assessment and Counseling: Conduct thorough psycho-social assessments of residents and clients, and provide counseling services to individuals, families, and groups as needed.
- Care Planning: Assist in the development, implementation, and review of resident and client care plans, ensuring that their needs are met and their goals are achieved.
- Discharge Planning: Participate in resident and client discharge planning, ensuring a smooth transition to community-based care.
- Education and Training: Develop and provide education programs for residents, clients, families, and staff, promoting health and wellness.
- Collaboration and Communication: Work collaboratively with healthcare teams, including physicians, nurses, and other healthcare professionals, to ensure comprehensive care.
- Education: Bachelor's Degree in Social Work from an accredited school of Social Work.
- Experience: Two years of recent experience working as a Social Worker in a long-term care setting, with clinical training and experience in individual, marital/family, and group modalities of social work practices.
- Registration: Current full registration with the BC College of Social Workers.
