Social Services Professional

Job Summary:
We are seeking a highly skilled and compassionate Social Worker to join our team at Fraser Health. As a Social Worker, you will play a vital role in providing psychosocial assessment and therapeutic clinical counseling services to clients in psychiatry. You will work collaboratively with our interdisciplinary team to develop and implement care plans that promote improved psychosocial and emotional well-being.
Key Responsibilities:
- Conduct thorough psychosocial assessments of clients through focused interviews and data interpretation.
- Develop and implement individualized care plans that address clients' unique needs and goals.
- Provide supportive counseling services to clients, including identifying areas of strength and challenge, and offering interventions to promote improved psychosocial and emotional well-being.
- Collaborate with the interdisciplinary team to plan for care during hospital stays and community treatment plans after discharge.
- Act as a liaison between psychiatric units and community agencies, facilitating referrals and providing information about access to community resources.
- Provide education and consultation to clients and healthcare professionals on issues related to Social Work practice in the psychiatric setting.
Qualifications:
- Bachelor's Degree in Social Work from an accredited school of Social Work.
- One year's recent, related experience.
- Current full registration with the BC College of Social Workers.
- Valid BC Driver's license and access to personal vehicle for business-related purposes.
Skills and Abilities:
- Demonstrated ability to conduct social and psychosocial assessments.
- Familiarity with DSM V criteria.
- Knowledge of counseling skills, including brief solution-focused therapy, crisis intervention, and resource counseling.
- Ability to work effectively with others, including working within a multicultural environment.
- Ability to make effective referrals, reflecting in-depth knowledge of community resources and strong collaborative skills.
