Job Summary:
We are seeking a highly skilled and compassionate Social Worker to join our team at Fraser Health. As a Social Worker, you will play a vital role in providing psychosocial assessments and therapeutic clinical counseling services to clients in psychiatry. You will work collaboratively with our interdisciplinary team to develop and implement care plans that promote improved psychosocial and emotional well-being.
Key Responsibilities:
- Conduct thorough psychosocial assessments of clients through focused interviews and data interpretation.
- Develop and implement care plans that address the unique needs of each client.
- Provide ongoing monitoring and evaluation of client progress, revising care plans as necessary.
- Collaborate with the interdisciplinary team to ensure seamless care coordination.
- Act as a liaison between psychiatric units and community agencies to facilitate referrals and resource linkage.
- Provide education and consultation to clients and healthcare professionals on social work practice in psychiatry.
- Participate in quality improvement initiatives and ongoing professional development.
Qualifications:
- Bachelor's Degree in Social Work from an accredited school.
- One year's recent, related experience.
- Current full registration with the BC College of Social Workers.
- Valid BC Driver's license and access to a personal vehicle for business purposes.
