Wellness Nurse

At Sunrise Senior Living, we are committed to providing high-quality care to our residents. As a Wellness Nurse, you will play a vital role in ensuring the overall well-being of our residents.
Responsibilities:- Conduct regular wellness visits for all residents to identify their ongoing needs and services.
- Collaborate with the Resident Care Director to evaluate and document changes in resident health care services and status.
- Assist in maintaining accurate and up-to-date resident wellness files.
- Communicate effectively with residents, families, and healthcare providers to ensure medical needs are met.
- Monitor and report any incidents or falls, and document accordingly.
- Participate in quality improvement initiatives to enhance resident care and services.
- Graduate of an approved college or school of nursing.
- Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
- Minimum of one year experience in home health, assisted living, hospital, or long-term care environment.
- Excellent communication and interpersonal skills.
- Ability to work weekends, evenings, and flexible hours.
