Wellness Nurse

At Sunrise Senior Living, we are committed to providing high-quality care for our residents. As a Wellness Nurse, you will play a vital role in ensuring the overall well-being of our residents.
Responsibilities:- Conduct regular wellness visits for all residents to identify their ongoing needs and services.
- Collaborate with the Resident Care Director to evaluate and document resident care plans.
- Monitor and follow up with residents who have experienced falls, accidents, or incidents, and document and report as required.
- Communicate with residents' families and responsible parties to ensure their medical needs are being met.
- Assist in maintaining wellness files according to Sunrise policies and federal and state regulations.
- Provide clinical support and assistance to community team members as needed.
- Graduate of an approved college or school of nursing.
- Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
- Minimum of one year experience in home health, assisted living, hospital, or long-term care environment.
- Demonstrated knowledge of nursing practices, techniques, and methods applied to health and wellness resident care, and federal, state, and local regulations.
