Responsibilities:- Conduct regular wellness visits for all residents to identify their ongoing needs and services.
- Collaborate with the Resident Care Director to ensure proper follow-through and evaluation of resident care services.
- Monitor and document resident incidents, falls, and accidents, and report as required.
- Maintain accurate and up-to-date resident wellness files.
- Communicate effectively with residents, families, and healthcare providers to ensure medical needs are met.
- Assist in maintaining medical supplies and emergency kits for the community.
- Provide clinical support and assistance to community team members as needed.
- Graduate of an approved college or school of nursing.
- Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
- Minimum of one year experience in home health, assisted living, hospital, or long-term care environment.
- Excellent communication and interpersonal skills.
- Ability to work weekends, evenings, and flexible hours.
