Wellness Nurse

At Sunrise Senior Living, we are committed to providing exceptional care and services to our residents. As a Wellness Nurse, you will play a vital role in promoting the overall well-being of our residents.
Responsibilities:- Conduct regular wellness visits for all residents to identify their ongoing needs and services.
- Collaborate with the Resident Care Director to ensure proper follow-through and documentation of resident care services.
- Assist in conducting resident evaluations and gathering data to inform care decisions.
- Monitor and follow up with residents who have experienced falls, accidents, or incidents, and document and report as required.
- Communicate effectively with residents' families and responsible parties to ensure their medical needs are being met.
- Work closely with attending physicians to ensure residents receive the best possible care.
- Maintain accurate and up-to-date resident wellness files, including weights and vital signs.
- Assist in maintaining medical supplies and emergency kits for the community.
- Provide clinical support and assistance to community team members as needed.
- Graduate of an approved college or school of nursing.
- Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
- Minimum of one year experience in home health, assisted living, hospital, or long-term care environment.
- Strong knowledge of nursing practices, techniques, and methods applied to health and wellness resident care.
- Excellent communication and interpersonal skills.
- Ability to work weekends, evenings, and flexible hours.
