Wellness Nurse

At Sunrise Senior Living, we are committed to providing high-quality care and services to our residents. As a Wellness Nurse, you will play a vital role in promoting the overall well-being of our residents.
Responsibilities:- Resident Care: Identify ongoing needs and services of residents, ensuring the highest quality resident services are delivered.
- Health Monitoring: Ensure proper follow-through, assessment, and documentation for residents with changes in healthcare services and status.
- Assessments: Assist Resident Care Director in conducting resident assessments at move-in, 30 days post-move-in, semi-annually, and with any change in status.
- Wellness Visits: Conduct monthly wellness visits for all residents, assessing, monitoring, and following up on any falls, accidents, or incidents.
- Communication: Maintain open communication with residents' families and responsible parties to ensure medical needs are met.
- Medical Support: Provide clinical support and assistance to community team members as needed.
- Education: Graduate of an approved college or school of nursing.
- Licensure: Current state license as a professional Registered Nurse (RN).
- Experience: Minimum of one year experience in home health, assisted living, hospital, or long-term care environment.
- Skills: Demonstrated knowledge of nursing practices, techniques, and methods applied to health and wellness resident care, and federal, state, and local regulations.
