About the Role

We are seeking a highly skilled and experienced Registered Dietitian to join our team as a Diabetes Educator. As a key member of our multidisciplinary team, you will play a vital role in providing comprehensive diabetes education and support to our patients.

Key Responsibilities
  • Diabetes Education and Support: Develop and implement individualized care plans for patients with diabetes, providing education and support on self-management skills, medication management, and lifestyle modifications.
  • Collaboration and Communication: Work closely with our multidisciplinary team, including endocrinologists, nurses, and other healthcare professionals, to ensure seamless care coordination and communication.
  • Program Development and Implementation: Assist in the development and implementation of new programs and initiatives to improve patient outcomes and satisfaction.
  • Training and Education: Provide training and education to other staff members on diabetes education and management.
  • Research and Quality Improvement: Participate in research studies and quality improvement initiatives to enhance patient care and outcomes.
Requirements
  • Education and Experience: A valid RD license and in good standing with the College of Dietitians of Ontario, with a minimum of 2 years of experience in diabetes education and management.
  • Skills and Qualifications: Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals. Proficiency in motivational interviewing and other evidence-based coaching techniques is an asset.
