About the Role

LMC Healthcare, a leading specialist care provider in diabetes and endocrinology, is seeking a highly skilled Diabetes Educator and Registered Dietitian to join our team on a temporary, full-time basis. As a key member of our multidisciplinary team, you will play a vital role in transforming diabetes care by making it more accessible, comprehensive, and patient-centric.

Responsibilities
  • Education and Patient Support: Educate and advise patients on self-management skills in individual and group settings, focusing on diabetes and secondary health conditions such as general health and metabolic conditions.
  • Care Planning and Collaboration: Develop care plans for patients and collaborate with the multidisciplinary team as needed.
  • Electronic Medical Records (EMR) Management: Complete detailed patient encounters within our EMR system.
  • Insulin Administration and Management: Teach patients the effective administration of insulin, including titration and management, as per medical directives.
  • Blood Glucose Monitoring: Teach patients self-monitoring of blood glucose (SMBG) effectively, as per medical directives.
  • Patient Follow-Ups: Perform patient follow-ups in person or remotely via email, telephone, or video.
  • Health History and Consultation: Obtain relevant health history of patients and provide consultation as requested to community groups, partners, and team members.
  • Training and Education: Train additional staff and new educators as required.
  • Communication and Technology: Effectively communicate with patients, families, caregivers, and healthcare professionals, and employ information technology appropriately for patient care.
  • Professional Development: Stay current and aware of opportunities to implement new, evidence-based methods of patient assessment and treatment.
  • Research and Program Implementation: Assist with research studies, including providing diet counseling for patients enrolled in research studies, and participate in program implementation.
  • Community Outreach: Complete a minimum of 2 community outreach events annually and participate in annual outreach calls.
  • Administrative Tasks: Assist with administrative tasks, including rescheduling follow-up appointments, creating workshop folders, ordering supplies and medications, tracking patient visits, and entering data for programs.
  • Professional Growth and Development: Participate in Journal Clubs and other group learning opportunities, and participate in disseminating the program vision.
  • LMC Initiatives: Participate and promote non-ministry funded LMC initiatives, including paid and non-paid services.
Requirements
  • Education and Licensure: A valid RD license and in good standing with the College of Dietitians of Ontario.
  • Experience and Skills: Experience teaching/counseling individuals and groups of people, and a proven track record of being resourceful and finding solutions even when there is no clear path.
  • Certifications and Designations: Must obtain or be working towards CDE designation.
  • Driver's License and Vehicle: Must have a valid driver's license in good standing and access to a personal vehicle.
