Community Navigator, Correctional Health Services

Job Summary:
We are seeking a highly skilled and compassionate Community Transition Team Systems Navigator to join our team at BC Mental Health & Substance Use Services. As a key member of our team, you will play a vital role in coordinating referrals to care, working with Indigenous partners, and facilitating holistic care planning for clients with complex needs.
Key Responsibilities:
- Coordinate referrals to care, including primary care and social supports, using a culturally informed and client-centered approach.
- Work with Indigenous partners and the FNHA to ensure the best interests of clients and families are met.
- Facilitate holistic care planning, engaging with and coordinating inputs from various service providers to support clients with complex needs.
- Participate in care coordination meetings and multidisciplinary case conferences as needed to support client access to care.
- Network with service providers and build collaborative relationships to support care planning.
- Provide brief supportive counselling and information services to clients, establishing trust and confidentiality.
Requirements:
- Master's Degree in Social Work or Behavioural Science from a recognized program.
- Minimum one (1) year of recent related clinical experience working with complex care individuals with significant mental health, substance use issues, and/or cognitive limitations.
- Demonstrated systems thinking skills and knowledge of community development, community health agencies, and services.
- Awareness of and commitment to learning and understanding the Truth & Reconciliation Commission's 94 Calls to Action (2015), In Plain Sight Report (2020), BC's Declaration on the Rights of Indigenous Peoples Act (2019), and other related reports.
