Indigenous Patient Navigator

We are seeking a highly skilled and compassionate Indigenous Patient Navigator to join our team at PHSA. As a key member of our Community Transition Team, you will play a vital role in providing consultative and navigational services to patients referred to our programs and services.
Key Responsibilities:- Provide cultural interpretation and health literacy translation services to support patients in accessing and navigating healthcare services.
- Support patients in communications with healthcare providers and other frontline care staff.
- Coordinate access and coordination of services for patients within the designated region.
- Advocate for the needs of patients within the healthcare system and liaise with other community and regional health organizations.
- Provide support and act as a resource for healthcare providers and other staff members regarding culturally safe care.
- Diploma or degree in a related health discipline, health or social sciences, or a patient care/clinical related discipline.
- 1-3 years' recent related experience working in Indigenous healthcare.
- Ability to travel and valid driver's license.
