Job Overview:
The Client Services Team Leader is a Registered Practical Nurse (RPN) tasked with overseeing the comprehensive care and service delivery for clients during their shift. This role includes the precise and timely administration of medications, treatments, documentation, and responding to emergencies. The Team Leader manages daily operations at the facility and supervises staff members, including Personal Support Workers (PSWs).
The Client Services Team Leader reports directly to the Client Services Manager and collaborates closely with SPRINT Senior Care personnel, caregivers, families, vendors, and service partners to guarantee high-quality services.
Key Responsibilities:
- Oversee the direct care provided to clients and manage unionized staff.
- Ensure that care is delivered in a safe and hygienic environment.
- Communicate with physicians, pharmacists, and other healthcare professionals regarding client care.
- Provide ongoing feedback and insights on clients to the Client Services Manager.
- Keep all parties involved in the care loop informed of any changes in the client's health status.
- Assist in formulating comprehensive care plans and ensure regular updates are made.
- Manage medication processes for each client, including ordering, verifying, and distributing medications, while maintaining accurate medication records.
- Train SPRINT Senior Care staff on delegated responsibilities and supervise adherence to medication protocols.
- Offer guidance, coaching, and support to staff concerning medical and healthcare inquiries.
- Conduct client assessments, including the interRAI CHA.
- Facilitate client transfers to and from medical facilities, ensuring timely communication of all relevant information.
- Act as a backup for Personal Support Workers when necessary.
- Provide on-call services as required.
- Ensure that the activities of the facility align with the mission, vision, and values of SPRINT Senior Care while complying with established policies and procedures.
- Engage in the development, implementation, and evaluation of quality and risk management initiatives within the department and agency.
- Participate actively in departmental, leadership, accreditation, and interdisciplinary teams.
Qualifications:
- Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario.
- A minimum of two years of experience in gerontology and/or community care settings.
- Strong leadership abilities, with 2-4 years of supervisory experience in a unionized environment.
- Exceptional communication and interpersonal skills, capable of building rapport with staff, clients, families, and referral sources in a diverse setting.
- Thorough understanding of aging, dementia, and the specific needs of the elderly, disabled, and their caregivers.
- Culturally sensitive to the needs of clients from various backgrounds.
- Self-motivated, results-driven, and able to work independently as well as collaboratively in a team.
- Excellent organizational, facilitation, strategic, and problem-solving capabilities.
- Strong knowledge of program evaluation, risk management, and quality improvement methodologies.
- Proven ability to foster and maintain productive relationships with internal and external stakeholders.
- Proficient in computer applications, particularly Microsoft Office Suite (Word, PowerPoint, Publisher, Excel, and Outlook) at an intermediate level.
- Intermediate understanding of database concepts and structures; familiarity with Alaya Care database is a plus.
