Job Title: Social Worker I

Fraser Health is seeking a highly skilled and compassionate Social Worker I to join our team. As a key member of our interdisciplinary team, you will provide therapeutic clinical counseling services to patients and families in our acute care settings.

Key Responsibilities:
  • Conduct social work assessments of patients and families using a variety of modalities and in accordance with professional standards and clinical policies and practices.
  • Provide consultation to patients, families, and healthcare professionals on issues related to high-risk factors, recovery, and current community practice relevant to the assigned clinical area.
  • Develop and coordinate care plans with the interdisciplinary team to ensure effective utilization of patient resources.
  • Collaborate with community health and social agencies to determine alternate levels of care and facilitate referrals to appropriate resources.
  • Provide education and support to patients, families, and healthcare professionals on social work practice and community resources.
Requirements:
  • Bachelor's Degree in Social Work from an accredited school of Social Work.
  • Current full registration with the BC College of Social Workers.
  • One year of recent, related experience in a hospital setting and/or public agency.
  • Comprehensive knowledge and understanding of emotional and mental health issues related to illness and injury, and patient/family cultural dynamics.
  • Basic counseling skills, including crisis counseling, brief therapeutic interventions, and loss and grief applicable to families, young adults, and seniors.
