We are seeking a highly skilled and experienced Clinical Social Worker to join our team at Fraser Health. As a key member of our interdisciplinary team, you will be responsible for providing comprehensive social work services to patients and families in our in-patient psychiatric unit.
Key Responsibilities- Conduct comprehensive social work and psychosocial assessments of patients suffering acute episodes of mental illness and substance misuse/dependence.
- Develop and coordinate care plans and community services for patients requiring facility placement and/or community support.
- Collaborate with hospital and community services to determine appropriate levels of care and the effective utilization of patient resources.
- Provide education and consultation to patients, families, and healthcare professionals on issues related to mental health, child protection, and community living.
- Participate in the development and delivery of in-services, presentations, and psycho-educational groups.
- Initiate, develop, and carry out research activities in collaboration with the interdisciplinary team.
- Master's Degree in Social Work from an accredited school of Social Work.
- Current full registration with the BC College of Social Workers.
- One year's recent, related experience in a mental health treatment setting.
- Ability to work effectively in a multicultural environment and as part of an integrated interdisciplinary team.
- Strong communication and interpersonal skills.
