We are seeking a highly skilled and compassionate Social Worker I, Acute Care to join our team at Fraser Health Authority. As a key member of our interdisciplinary team, you will provide therapeutic clinical counselling services to patients and families in the assigned clinical area within Acute Care.
Key Responsibilities:- Conduct psychosocial assessments of patients and families to identify problems, personal concerns, and treatment objectives.
- Provide consultation to patients, families, and healthcare professionals on issues related to high-risk factors, recovery, and current community practice.
- Develop, coordinate, and deliver in-services, presentations, and support groups to promote patient and family well-being.
- Lead the discharge planning process, ensuring effective utilization of patient resources and coordination with hospital and community services.
- Communicate effectively with community health and social agencies to facilitate referrals and access to community resources.
- Bachelor's Degree in Social Work from an accredited school of Social Work.
- Current full registration with the BC College of Social Workers.
- Comprehensive knowledge and understanding of emotional and mental health issues related to illness and injury.
- Basic counselling skills, including crisis counselling, brief therapeutic interventions, and loss and grief.
- Ability to work effectively in a multicultural environment and as part of an interdisciplinary team.
