About the Role:
We are seeking a highly skilled and compassionate Social Worker I to join our team at Fraser Health. As a key member of our interdisciplinary team, you will provide therapeutic clinical counselling services to patients and families in our acute care settings.
Key Responsibilities:
- Conduct social work assessments of patients and families using a variety of modalities and in accordance with professional standards and clinical policies and practices.
- Provide consultation to patients, families, and healthcare professionals on issues related to high-risk factors, recovery, and current community practice relevant to the assigned clinical area.
- Develop, coordinate, and deliver in-services, presentations, and support groups to promote patient and family education and well-being.
- Collaborate with hospital and community services to determine alternate levels of care and the effective utilization of patient resources.
- Act as a resource to patients and their families by providing information and referrals to community resources and services.
Requirements:
- Bachelor's Degree in Social Work from an accredited school of Social Work.
- Current full registration with the BC College of Social Workers.
- One year of recent, related experience in a hospital setting and/or public agency.
