Key Responsibilities- Conduct thorough assessments to identify clients' needs and goals
- Develop and implement personalized treatment plans to address clients' physical, cognitive, social, emotional, and spiritual needs
- Collaborate with a multidisciplinary team to provide comprehensive care and support
- Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination

- Graduate of an accredited program in Occupational Therapy and a member in good standing of the College of Occupational Therapists of Ontario
- Recent experience in managing patients in acute/rehab or community-based rehabilitation programs

- Ability to adapt to new computer applications and technology
