Key Responsibilities- Conduct thorough assessments to identify clients' physical, cognitive, social, emotional, and spiritual needs
- Develop and implement client-centered, goal-directed treatment programs to address identified needs
- Collaborate with interdisciplinary rehabilitation teams to provide comprehensive care
- Communicate effectively with clients, families, and healthcare professionals to ensure seamless care transitions

- Ability to adapt to new computer applications and work effectively in a team environment
