Job Title: Community and Social Services Worker

We are seeking a dedicated and compassionate Community and Social Services Worker to join our team at Improving Lives In-Home Care. As a key member of our team, you will play a vital role in providing support and care to individuals in their homes.

Job Summary:

The Community and Social Services Worker will be responsible for providing assistance with daily living activities, such as housekeeping, food preparation, and personal hygiene. You will also work closely with clients to develop and implement individualized care plans, ensuring their physical, emotional, and social needs are met.

Key Responsibilities:
  • Assist clients with daily living activities, including housekeeping, food preparation, and personal hygiene
  • Develop and implement individualized care plans to meet clients' physical, emotional, and social needs
  • Provide companionship and social support to clients
  • Assist clients with special needs, such as bathing and dressing
  • Prepare reports and case histories as required
  • Work collaboratively with other healthcare professionals to ensure comprehensive care
Requirements:
  • Secondary (high) school graduation certificate
  • 1 year to less than 2 years of experience in a related field
  • Ability to work independently and as part of a team
  • Physical stamina to perform tasks that may be physically demanding
