We are seeking a highly skilled Registered Nurse to join our Renal Services team at Fraser Health Authority. As a key member of our inter-professional care team, you will provide direct patient care, assess patient needs, and develop and implement individualized care plans.
Responsibilities- Conduct comprehensive nursing assessments of patients on admission and at regular intervals, identifying risk factors and developing nursing diagnoses.
- Collaborate with patients, families, and healthcare teams to develop and implement care plans, ensuring patient-centered care and promoting autonomy and choice.
- Provide direct patient care, including administering medications, performing procedures, and providing education and support to patients and families.
- Participate in quality improvement initiatives, identifying areas for improvement and contributing to the development of evidence-based practices.
- Communicate effectively with patients, families, and healthcare teams, using verbal and written communication skills to ensure seamless care transitions.
- Graduation from an approved School of Nursing and current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives.
- Two years' recent, related clinical nursing experience in the designated clinical area, with completion of post-graduate coursework and/or certification applicable to the designated clinical area.
- Clinical proficiency in the designated clinical area, with ability to initiate and maintain intravenous therapy and recognize acuity changes.
- Ability to use factual information, prior learning, and basic principles and procedures to support nursing decisions and actions, with relevant research-based evidence/evidence-informed practice.
- Ability to communicate effectively with patients, families, and healthcare teams, using verbal and written communication skills to ensure seamless care transitions.
