Licensed Practical Nurse, Hemodialysis Specialist

Job Summary:
We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our hemodialysis team at Fraser Health. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our renal program.
Key Responsibilities:
- Assess, plan, implement, and evaluate patient care during the pre-dialysis, intra-dialysis, and post-dialysis periods.
- Establish and maintain a therapeutic relationship with patients and their families through effective communication and interpersonal skills.
- Contribute to the development and ongoing changes to individualized care plans, including attending multidisciplinary care planning meetings and family conferences.
- Advocate for patients' interests and needs, ensuring their autonomy and choice in decision-making and care planning.
- Perform patient care duties in collaboration with registered nurses and other members of the nephrology team.
- Escalate care as required for changes in patients' conditions.
- Participate in quality improvement and program activities to maintain consistent levels of care and ensure interdisciplinary team members work collaboratively.
Qualifications:
- Grade 12, plus graduation from an approved program of Practical Nursing, and successful completion of post-basic education through an in-house program offered by a health authority affiliated with BC Renal.
- Current full practicing registration with the British Columbia College of Nurses and Midwives (BCCNM) and current certificates in CPR Level C or equivalent and First Aid.
