We are seeking a highly skilled Mental Health Social Worker to join our team at Southlake Regional Health Centre. As a key member of our Mental Health Team, you will be responsible for providing exceptional patient care and support to individuals with serious mental illness.
Key Responsibilities- Conduct comprehensive assessments and develop treatment plans for clients in the Adult Outpatient Mental Health program.
- Provide short-term counseling and psycho-social support to individuals and groups, utilizing evidence-based modalities such as Brief Solution Focused model, cognitive behavioral, and dialectical behavioral therapy.
- Document clinical assessments, treatment plans, and client progress, ensuring accurate and timely reporting to the team.
- Liaise with team members, hospital staff, and community agencies to promote effective collaboration and communication.
- Develop and lead client groups, demonstrating expertise in group dynamics and facilitation skills.
- Collect and analyze statistical data to inform program development and improvement.
- Participate in committee membership and contribute to the development of policies and procedures.
- Masters in Social Work (MSW) degree.
- Three to five years of experience working with seriously mentally ill clients.
- Registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
- Advanced counseling and assessment skills, with proficiency in various short-term counseling modalities.
- Excellent communication, analytical, and problem-solving skills.
- Ability to work collaboratively in an interprofessional team environment and independently.
- Regular attendance and commitment to customer service and teamwork.
