Job Summary

We are seeking a skilled and compassionate Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our team as a Community Nurse. As a Community Nurse, you will work with patients in their homes, clinics, and other community settings, providing exceptional nursing services and developing meaningful relationships with patients and their families.

Key Responsibilities
  • Develop and maintain effective care plans for patients receiving community health services
  • Provide nursing services to patients in their homes, clinics, and other community settings
  • Communicate and liaise with patients, families, and other healthcare professionals to ensure seamless care coordination
  • Perform other clinical duties as required
Requirements
  • Current registration with the College of Nurses of Ontario (CNO)
  • Strong interpersonal and communication skills
  • Valid driver's license and vehicle
  • CPR certification
  • Current immunization record, including 2-step TB test
  • Vulnerable sector check and clear background check
