CarePartners is seeking a skilled Nurse to join our team as a Community Nurse. As a Community Nurse, you will have the opportunity to develop meaningful relationships with patients and provide exceptional nursing services in a variety of settings.
Responsibilities- Develop and maintain effective Care Plans with patients and their families.
- Provide nursing services in-home, clinic, retirement home, or school settings.
- Communicate and collaborate with interdisciplinary care teams.
- Perform other clinical duties as required.
- Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma.
- Current College of Nurses of Ontario (CNO) registration in good standing.
- Strong interpersonal skills and a commitment to patient-centered care.
- Valid driver's license and vehicle.
- Excellent English verbal and written communication skills.
