About the Role

CarePartners is seeking a skilled Nurse to join our team as a Community Nurse. As a Community Nurse, you will have the opportunity to develop meaningful relationships with patients and provide exceptional nursing services in a variety of settings.

Responsibilities
  • Develop and maintain effective Care Plans with patients and their families.
  • Provide nursing services in-home, clinic, retirement home, or school settings.
  • Communicate and collaborate with interdisciplinary care teams.
  • Perform other clinical duties as required.
Requirements
  • Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma.
  • Current College of Nurses of Ontario (CNO) registration in good standing.
  • Strong interpersonal skills and a commitment to patient-centered care.
  • Valid driver's license and vehicle.
  • Excellent English verbal and written communication skills.
