Key Responsibilities
  • Develop and maintain effective Care Plans with respect to Community Health Services, independently or in cooperation with the Ontario Health atHome.
  • Provide exceptional nursing services to patients, both in-person and virtually.
  • Communicate and liaise with patients, family members, and others within the interdisciplinary care team.
  • Perform other clinical duties as required.

Requirements

  • Exceptional interpersonal skills.
  • A valid driver's license and vehicle as regular travel is required.
  • Excellent English verbal and written communication skills.
  • CPR Certification.
  • Current immunization record including 2 step TB test.
  • To protect our patients, we require a current vulnerable sector check and a clear background check.
