Key Responsibilities- Develop and maintain effective Care Plans with respect to Community Health Services, independently or in cooperation with the Ontario Health atHome.
- Provide exceptional nursing services to patients, both in-person and virtually.
- Communicate and liaise with patients, family members, and others within the interdisciplinary care team.
- Perform other clinical duties as required.

- A valid driver's license and vehicle as regular travel is required.
- Excellent English verbal and written communication skills.
- CPR Certification.
- Current immunization record including 2 step TB test.
- To protect our patients, we require a current vulnerable sector check and a clear background check.
