Job Summary

We are seeking an experienced Occupational Therapist to join our team as a Home HealthCare Specialist. As an Occupational Therapist with VHA, you will work with diverse client groups across Southern Ontario and the capital region, providing consultation, assessment, and planned interventions to support their independence and well-being.

Key Responsibilities
  • Assess and implement occupational therapy interventions in accordance with professional practice guidelines and standards established by VHA and the relevant regulatory college.
  • Develop and implement client-centered care plans in collaboration with the client's integrated care team.
  • Ensure coordinated delivery of care through ongoing communication with all members of the client's team regarding plans and schedules for interventions.
  • Document all assessment findings, treatment plans, progress, and discharge summaries of interventions as required by the profession, referral source, and the organization.
  • Support safe client care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.
Requirements
  • Master's degree in Occupational Therapy from a recognized school of Occupational Therapy.
  • Member in good standing with the College of Occupational Therapists of Ontario.
  • Valid Driver's License with access to a vehicle.
  • Compassionate and empathetic individual with excellent communication and interpersonal skills.
  • Self-driven with ability to work independently and as part of a team.
  • Skilled in caseload management and electronic documentation.
  • Able to provide virtual care and support.
  • Registered ADP authorizer (asset).
