Key Responsibilities- Assess client needs and develop personalized care plans in collaboration with healthcare professionals and family members.
- Coordinate and implement care services, including home care and placement, to meet client needs and preferences.
- Provide education and support to clients and their families on healthcare options and community resources.
- Collaborate with healthcare teams to ensure seamless transitions and continuity of care.
- Monitor and evaluate client progress, making adjustments to care plans as needed.
- Registered Nurse (BScN or diploma) or equivalent degree in a related field.
- Minimum 2 years of experience in healthcare, preferably in a community care setting.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
- Proficiency in the use of computerized processes and mobile technology.
