Job Summary:

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Home and Community Care Support Services. As a Care Coordinator, you will play a vital role in managing a caseload of diverse patients, assessing their care needs, and developing personalized care plans that cater to their unique health and psychosocial requirements.

Key Responsibilities:

  • Conduct comprehensive patient assessments to identify clinical, physical, and psychosocial needs, as well as caregiver needs.
  • Assess patient and caregiver understanding of diagnosis, treatment, and available resources, and provide education and support as needed.
  • Collaborate with patients, caregivers, and healthcare teams to develop and implement individualized care plans that address patient priorities and desired outcomes.
  • Coordinate the delivery of Home and Community Care services, including medical equipment, supplies, and services.
  • Facilitate communication and cooperation among healthcare team members to ensure seamless care coordination.
  • Provide regular reports to physicians, service providers, patients, and caregivers as required.

Requirements:

  • Registered Nurse (BScN or diploma) or equivalent degree in a related field (e.g., Physiotherapy, Occupational Therapy, Social Work, Speech Therapy, or Foods and Nutrition).
  • Related professional experience, preferably in a community-based setting.
  • Knowledge of medical diagnoses, prognoses, symptom management, and cognitive/psychiatric disorders.
  • Knowledge of case management principles and best practices.
  • Knowledge of resource availability, service costs, and budgetary parameters.
  • Knowledge of relevant healthcare legislation.

Preferred Qualifications:

  • Fluency in English and French.
  • Valid driver's license and access to a vehicle.
  • Vulnerable Sector Check completed within the last six months.
