In this role, you will work closely with clients to assess their needs, develop personalized plans, and provide support to help them achieve their goals. You will also collaborate with other social services agencies and healthcare providers to ensure a comprehensive approach to client care.

Key Responsibilities:

* Assess client needs and develop personalized plans

Requirements:

* Bachelor's degree in Social Work or a related field
* 1-2 years of experience in social services or a related field

* Strong problem-solving and critical thinking skills
* Ability to maintain confidentiality and handle sensitive information
