Job Title: Social Worker - Mental Health Outpatient Program

Job Summary:

We are seeking a highly organized and skilled Social Worker to join our interprofessional team at St. Joseph's Care Group. The successful candidate will work in our Mental Health Outpatient Program, providing assessment, treatment, and support to individuals with serious and persistent mental illness.

Key Responsibilities:
  • Assess and treat individuals with serious and persistent mental illness
  • Provide individual and group counseling, as well as motivational interviewing and solution-focused methods
  • Work collaboratively with an interprofessional team to provide comprehensive care
  • Develop and implement treatment plans that address the unique needs of each individual
  • Document all interactions and interventions in electronic medical records
Requirements:
  • Masters of Social Work Degree and current registration with the OCSWSSW
  • Knowledge and experience with referral processes and triage, decision guide processes, and system navigation
  • Training and experience in motivational interviewing and solution-focused methods, principles in self-management and strengths-based interventions
  • Experience/training in a variety of evidence-based therapies, including solution-focused therapy, EMDR, CPT, CBT, DBT, and group therapies
  • Knowledge and skills in a range of theoretical perspectives, psychosocial rehabilitation principles, self-management, and holistic wellness approaches
  • Excellent oral and written communication and interpersonal skills, judgment, critical thinking skills, problem-solving, and time management skills
Working Conditions:
  • This position requires a Police Vulnerable Sector Check (PVSC) and proof of COVID-19 vaccination or valid medical exemption
