Job Title: Client Care Navigator

Job Summary:

The Client Care Navigator will play a vital role in coordinating and integrating supports and services around individuals living with vision loss. This direct client service role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Key Responsibilities:
  • Coordinate and integrate supports and services around individuals living with vision loss
  • Assess client needs and develop personalized care plans
  • Strengthen communication and linkages between VLR staff and external community healthcare providers
  • Provide timely access to VLR service information and connection to the healthcare system
  • Enhance client experience and wellbeing
  • Improve client capacity to remain independent
Requirements:
  • 3-5 years of client service experience in the health and/or social service sectors
  • Experience working directly with individuals with vision loss
  • Knowledge of available community services and clinical, social, and residential care options
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Proficiency in technology, including Microsoft Office and case management systems
